By agreeing to these terms, I, the parent/guardian, consent to the above named to participate in various on-ice activities conducted by Transform Sports Training & Rehabilitation. By accepting these terms, I, the parent/guardian, release Transform Sports Training & Rehabilitation from any liabilities resulting from injuries or accidents that may have occurred to the above named participant. By accepting these terms, I, the parent/guardian, will not hold responsible for any loss of equipment or injuries that may result in the performance of any activities either on-ice or off-ice to the above named participant. Each player must wear full CSA approved equipment.
Refunds/Credits
Refund and credit policies are subject to final approval from Transform Sports Training & Rehabilitation. Written notice of cancellation is measured from the original start date of the camp/clinic. Written notice must me presented via email or by registered mail to the address stated on the contact page.
Full Refund: Minimum 30 day notice, subject to 15% administration fee
50% Credit: 15-30 days notice
14 days or less: no refund (may be eligible for review)
